Dewan Khan, 35, from Newbury, Berkshire, was a passenger in a red Peugeot travelling on the northbound M40 when the accident happened.

Warwickshire Police said the car somehow left the carriageway between junctions 12, Gaydon, and 13, Bishops Tachbrook.

Two more vehicles were then involved in Monday morning's accident.

The driver and another passenger in the Peugeot were taken to hospital with serious injuries.

A Jaguar driver suffered minor injuries.
